Please arrive at the time specified during your reminder call

New Clients:  Return New Client Form with all 5 pages completed

Do not arrive to pick up your pet until after you are called

Payment and Post-op instructions will be given when you pick up your pet

2. Turn in the form at Reception Desk & Sign-In 2. Get Pager--Works in retail store and parking lot
3. Get a Pager--Works in retail store and parking lot 3. Leave Clinic until Buzzed

Walk-In Check-In Rules

Your Pet may NOT be checked in unless they have been seen in the past 3 years 
or you have COMPLETED a new patient form
 
BE SPECIFIC
your pet will only be seen for procedures requested at check-in
 
Vaccinations--list types 
Heartworm test 
Flea/Tick/Heartworm preventative
Illness/Injury-list symptoms 
Etc.
 
Then at the very bottom of that page under the appointment heading add
  • A $35 fee must be paid before you can make an appointment if you have missed a previous appointment with less than 24 hours' notice
  • Two missed appointments result in being put on a no appointments allowed list
We have to start cracking down on no-call; no-shows and make sure people understand that the vet needs to know why they are here immediately, not spend 10-15 minutes trying to figure out why the client walked in with their pet for vet services. This is how we are hoping to continue helping the most people possible at a reasonable cost

2. Turn in the form at Reception Desk & Check-In 2. Once Called enter clinic--only 2 people per party
3. Get a Pager--Works in retail store and parking lot  
4. Leave Clinic until Buzzed  
5. Once Buzzed enter clinic--only 2 people per party
